Add labels to form fields in Squarespace

1. Edit the page with the form, click the form block's pencil icon, and open each field.
2. Make sure every field has a Label. Don't leave a field with only placeholder text.
3. For newsletter blocks, check that the email field has a label or placeholder setting in the block's options, and fill it in.
4. Forms added with a code block or a third-party embed need their labels fixed in that code (a <label for="..."> tied to each input's id), or by the embed's provider.

Check it: publish your change, then run a new scan from your dashboard. The issue should drop off the list.
